Fertility With Dr Felipe Lazar – Question From Readers

Can men with low sperm production be parents and without artificial insemination or IVF interventions just taking vitamin supplements?

There are few treatments for male fertility factors for a man who has no amount of sperm. Fertility treatments are available for men who have a limited amount of sperm and are able to undergo some type of treatment to restore fertility and a relatively good amount of material to achieve pregnancy. Antioxidants, vitamin E, vitamin C, help to decrease the fragmentation of the sperm shape and thus improve sperm quality, however for more severe cases such as absence of sperm or absence of motility, there is no solution.

For a satisfactory result, the time the couple is trying to conceive has and must be taken into account. If the couple has been trying for a long time and has no apparent cause on the female side, the chances of a pregnancy can be much higher. Inducers for sperm production may not work, but there are:

  • Aromatase inhibitors
  • Antioxidants

However, they may not be useful if the male material (sperm) is absent or is not in a condition to be recovered. Men who use anabolic steroids may have fertility problems as excess testosterone can decrease the amount of sperm. The man is able to produce sperm and still be fertile even at the age of 80, while the woman in the fertility race can be far behind and at 38, 40 is considered “old” to have children by the age of the eggs that produced. Remembering that the woman is born with a certain number of eggs and these are used every cycle during life if she does not use any contraceptive method. At 35 years old the woman has approximately 12% of the eggs she was born with and there are a total of 16,000, and at 40 this number can drop to just 3%.

What are the chances of Success in the First Fertilization Attempt?

On the medical side, the best chance for fertilization is on the first attempt. When the treatment is initial, whatever the causes of infertility are, such as tubal ligation, endometriosis and others, the first attempt gives chances of 60, up to 70% of success in the fertility procedure for couples with good conditions to achieve already in the first. Fertility treatment like IVF or ICSI, has to be fully timed, for the embryo to be transferred at the perfect time to the uterus. In the event that the uterus has become unsuitable for transfer due to time, the embryo can be frozen and then, in the next cycle with conditions necessary for the transfer and then the pregnancy happens. Usually the first time of fertility treatment is more effective, so the second and third attempts can be classified as 60, 40 and 20% of working out, of course everything will depend on the laboratory and the professionals involved and if there are conditions for the couple, certainly one of the three can work.

What are the fertility treatments if the woman does not have the tubes or the man is vasectomized?

When the possibility of sperm reaching the egg is taken away, other ways of helping with fertility must be adopted then, ICSI or In Vitro Fertilization are the best solutions. If the couple is young and the vasectomy or tubal ligation is recent, reversal can be attempted . If the couple is more mature, then the best way is to aspirate the man’s sperm or collect the woman’s eggs for artificial fertilization. In the case of lacquered women who had their tubes cut, the chances are much less, as the tube is a fundamental part of natural pregnancy, so referral to fertilization or ICSI is ideal.

Message from Dr Felipe to women who want to get pregnant.

“The age factor is fundamental, paying close attention to checking reproductive health to find out if there are any reproductive health problems such as endometriosis or another problem that could make pregnancy difficult. If the woman is healthy, pay close attention to her age, as age has no treatment , the woman has to try to be a younger mother in order not to have any major problems or if there is any problem, have enough time to revert and get pregnant naturally. Planning is essential and using the pill can keep the eggs young. ”
