What is Bolsa Rota and how to identify it?

We always see in movies, soap operas, or even with a friend or close relative, that the bag broke and ready, labor started. But it is not always that easy, there are many problems that can happen in this path and one of them is the broken bag.

What is Bolsa Rota?

First, I’ll explain what the amniotic bag is. In pregnancy, the fetus is wrapped in a membrane inside the uterus that contains a liquid produced by the mother’s body. This liquid protects the baby from impacts and possible problems with infection and it is there that the baby grows and develops with the nutrition of the placenta. When that membrane breaks without being completely, and thus a ruptured pouch is made.

The broken bag is in many cases sneaky. She can let all the amniotic fluid escape without the woman realizing that this is the reason for so much vaginal moisture. Often, the first-time mom may complain to the obstetrician and say that this humidity is normal. However, it is necessary that there is an attitude on the part of the doctor.

The ideal is to investigate the humidity in case of being raised to the point of wetting the clothes as was my case. The ruptured pouch can facilitate the penetration of bacteria that are the cause of infections in the fetus and consequently in the mother, or it can be the other way around. The mother who already has some type of infection, can pass this on to the baby. There is no way, at the slightest sign of rupture of the bag, no matter how small the fissure, delivery has to happen if it is a medical indication for your case .

How to Treat a Broken Bag

The broken pouch may or may not have labor present and it should be treated with antibiotics if the delivery period is longer than 24 hours. In fact, any case with ruptured pouch or total rupture should not exceed 24 hours to be treated in order to prevent infection .

Serious Cases

The risk of infection can be more serious in cases of short gestational age 24, 25 weeks for example. Although it can happen at any time of pregnancy, it is still too early for the baby to be born, after all, there are still 6 months of gestation. In these cases, the risk of the baby being inside the mother’s belly for much longer than the limit, can worsen with infections, since it is still very small and fragile.

Changing Diapers: Dr Jaime, how long can the bag be broken, I do not mean to break and leave all the water, but to be leaking. Does premature delivery have to happen with minimal leakage?

Dr Jaime: Broken bag, or amniorrexis, is the term used for spontaneous rupture of the amniotic bag, which can happen before or during labor. When it happens before labor, we call it Premature Amniorrexis, regardless of gestational age. The time that the bag can be broken depends a lot on the clinical picture and gestational age, since after 37 weeks the conduct is to perform the delivery (induction of labor or cesarean section, depending on the case).

However, if the pregnancy is still premature (less than 37 weeks ), and other complications are ruled out, the expectant conduct is chosen, that is, wait at least proximity to 37 weeks. For example, if the pouch breaks during a 32-week pregnancy, and it is found that there is no maternal infection, fetal distress or other changes, the pregnancy should be maintained for as long as possible up to 36-37 weeks.
